Mercedes Benz repair: 1974 450 SEL 4.5 rough idle, smoke like a train, pvc valve

Question
When I start the car it idles a little rough, then surges (revs) on its own till it is warmed up.  I've done a tune up (plugs/wires) and it helped the over all running but not the cold starts, is it the warm up regulator or a timing issue?

How is the power, like a rocket or rather weak?
With the engine running, if you remove the oil fill cap, does it puff smoke like a train?
Have you replaced the air breather Hoses?
Have you replaced the ignition coil?
