Trucking: Bid/Proposal Forms, dear mr, different places

Dear Mr. Bowers,
Just want to start off again and thank you for your great advice. My question is there anywhere I could look at sample/templates of bid/proposal forms to send out to the decision makers after I have identified them? I've researched a lot of different places and I can not find anything at all. Is there a generic bid/proposal form that is normally used around our industry? Any assistance would be greatly appreciated. Thanks again.

Manuel, any templates I have seen are created for a specific customer bid / response.  Many bids will come with their own response templates and some will simply ask for freeform responses to specific questions.  Sorry there is nothing I have seen that would be helpful.
